I refer to years ago where a major "production" defect in a networked military tactical data system - a system linking all the armed forces together. This high-visibility critical/major defect would blind all linked users by effectively wiping out all links and the shared contact information. Contacts are/were items being tracked via various sensors. Contacts usually consisted of submarines, surface vessels, and aircraft. All were further categorized as friendly, unknown or hostile with corresponding display symbology. It took 2 weeks to reproduce this and get at the root cause. The root cause was simply a table overrun which trounced the shared contact tracking table. The "rush to production" further contributed as the rush caused many tests to be omitted.
