Turns out permissions are missing. For each share the document requests you make, you need to add NT AuthoritySystem with Read/Write permissions (Package$ in particular). Turns out the file does not always update even though you may have updated the settings via the IIS manager. The steps here fixed the issue I was experiencing in my SCCM 2007 SP2 R2 instalaltion with Windows Server 2008 R2. http://techblog.mirabito.net.au/?p=269
